Connecting our community to the care they need

The Community Health Representative (CHR) Program at RVIHC provides outreach and transportation services to help community members access the health care they need. Our CHRs serve as a vital link between the clinic and the community, assisting patients who face barriers to care.

Outreach Services

Our Community Health Representatives conduct outreach throughout the Round Valley community to connect residents with health services, health education, and support programs.

  • Home visits for patients with limited mobility
  • Health education and prevention outreach
  • Assistance navigating health care and social services
  • Follow-up support after clinic visits
  • Community health screenings and events
  • Connection to behavioral health and social services

Transportation Services

RVIHC offers transportation assistance to eligible patients who need to travel to medical appointments both locally and outside the service area. Transportation services are coordinated through the Contracted Health Services (CHS) program for referred specialty care.

  • Local transportation to RVIHC appointments
  • Transportation coordination for CHS-referred specialty care
  • Assistance for patients without personal transportation
  • Scheduling support for out-of-area medical appointments
